ADOPTION CONTRACT
This Agreement and the exhibits attached hereto contain the entire agreement of the parties with respect to the subject matter of this Agreement, and supersede all prior negotiations, agreements and understandings with respect thereto. This Agreement may only be amended by a written document duly executed by all parties.

IMPORTANT: Please read the following and then initial each box indicating that you understand and agree to the terms of this contract. Any changes to the official printed language of this form shall be void.
I understand that all animals require ample time to adjust to new homes after adoption.
I agree that I will keep this pet as a part of my family, I will not make this dog an outside animal.
I agree to maintain appropriate Vet care, vaccinations, heartworm preventive and all other procedures recommended by my Vet or required by law.
I agree that I will not sell, give away or otherwise dispose of my adopted pet to any person, dealer, retailer, auction, institute or any other entity for any reason.
If for any reason I am unable to keep my adopted pet, I agree to contact Help A Hound, Inc. Help A Hound, Inc. has the right to reclaim this pet at no charge, and without refunding my adoption donation.
I recognize that upon delivery the dog I have adopted is healthy and has all Vet records and health Certificates from a Licensed Massachusetts Vet. I recognize that all animals can be carriers of various diseases, which can affect the health of other animals in my home, and for which Help A Hound, Inc. has no liability. I acknowledge that I am responsible for the payment of all medical costs and veterinary care that my adopted pet requires after adoption, and that Help A Hound, Inc. makes no representations or warranties, express or implied, as to the future health of my pet.
I recognize that Help A Hound, Inc. also makes no representations or warranties, express or implied, about my adopted pet's temperament or the suitability of this pet to me and/or any other person, and is hereby absolved from any liability for future damages or injuries caused by this animal.
This Agreement is a legally binding Contract and shall be governed by and construed in accordance with the laws of the Commonwealth of Massachusetts. Any legal action, motions, or proceeding arising out of or relating to this Agreement shall only be instituted in the County of Worcester, in the Commonwealth of Massachusetts.
It is the policy of Help A Hound, Inc. that all pet adoptions are based solely on the mutual best interest of both the pet and the prospective owner. Any intentional misrepresentation by you on this form will disqualify you from the adoption program.
I agree to allow Help A Hound, Inc. to reasonably and periodically check up on the pet for a 6 month period after the adoption to ensure the smoothest transition possible.
I hereby understand and agree that if Help A Hound, Inc. discovers that I have not complied with the terms of this Agreement at any time, that it may permanently confiscate my adopted pet from my care without refund of my adoption donation.
If any provision of this Contract is held unenforceable, then such provision will be modified to reflect the parties' intention. All remaining provisions of this Contract shall remain in full force and effect.
